    This form allows you to make your request for Driver's Safety Course Dismissal, but does not guarantee you that you will be granted this option. Your request will be reviewed by Clerk of the Court staff and you will be contacted by email or by phone once a determination has been reached on your eligibility. If you are granted Driver's Safety Course, a DSC Affidavit will be emailed to you along with your payment requirements. You will have to sign and email the DSC Affidavit back to the court and pay the DSC fee. You will also need to upload a COPY OF YOUR CURRENT TEXAS DRIVER'S LICENSE and CURRENT VALID INSURANCE showing you as a listed driver with your online request.
