Teen 2022/2023 School Year Volunteer Application

  1. Submit an application for the 2022/2023 School Year Volunteer Program between August 2022 - May 2023.

    Use this form to apply to be a teen volunteer at the library. The volunteer program runs between August 2022 and May 2023. 

    Teens earn community service hours for participating.

  6. Program Terms of Participation*
    • If I am selected as a Teen Volunteer, I will abide by al the rules of the DeSoto Public Library and the Teen 2022/2023 School Year Volunteer Program.
    • I understand that I am offering to volunteer free of charge.
    • I also understand that if I do not abide by all the rules of the DeSoto Public Library and the Teen 2022/2023 School Year Volunteer Program, that I will be dismissed from the program immediately.

    Medical Emergencies Involving Minors

    In the event that a parent or legal guardian cannot be reached in a medical emergency, the DeSoto Public Library is authorized to arrange for emergency medical treatment, the cost of which will be the sole responsibility of the parent or legal guardian.

    In case of emergency, please provide 3 contacts. Include first and last name, relationship, and phone number.

  10. I authorize the DeSoto Public Library to arrange emergency medical treatment, in case of an emergency. I understand that my son/daughter will not receive monetary compensation or be insured by the library.

    The Volunteer Coordinator will select applications.

    Selected volunteers must abide by the Teen 2022/2023 School Year Volunteer Program attendance policy, dress code and expected work conduct, and submit a background check. Arrangements for the final work schedule will be made by the Teen Volunteer Coordinator.

    There are only a limited number of volunteer hours and positions available! Therefore, all teens that apply are not guaranteed placement in the program.
