This form allows you to make your request for Driver's Safety Course Dismissal, but does not guarantee you that you will be granted this option. Your request will be reviewed by Clerk of the Court staff and you will be contacted by email or by phone once a determination has been reached on your eligibility. If you are granted Driver's Safety Course, a DSC Affidavit will be emailed to you along with your payment requirements. You will have to sign and email the DSC Affidavit back to the court and you will have to pay your fee's over the phone with the clerk. You will also need to attach with your online request, a COPY OF YOUR TEXAS DRIVER'S LICENSE and CURRENT INSURANCE showing you as a listed driver.
